In the first episode of season 10, Martin is forced to admit that he misses being a doctor when the mother of one of Louisa's child counselling patients presents with worrying symptoms but refuses to let him examine her. In episode two, the Portwenn surgery reopens, but chaos ensues as Martin and Louisa share an office and a less-than-competent new receptionist struggles to manage two sets of patients. Things are made worse when Louisa's childhood dream home goes on the market, and the accident-prone widower owner seems haunted by his wife's death—perhaps literally.

In episode three, Martin runs into former patient Stewart James, a park ranger who used to suffer from delusions that he lived with a giant squirrel. Stewart invites PC Penhale and Al Large to join him in the woods for a test run of his new survival course business, but when Stewart starts acting erratically, Al worries he's having a mental breakdown and must find a way to get help.

In episode four, Martin has his own headaches when an herbalist sets up shop in the village and a former flame arrives on his doorstep, inviting him to speak at a medical conference in London. Meanwhile, an ex-fiancée of PC Penhale's comes to town, giving him a chance to rekindle their romance.

In episode five, Louisa is shocked when her estranged father, Terry, makes an unannounced visit. Terry agrees to let Martin examine him, but this backfires on the Doc, who is now bound by patient confidentiality. Ruth, Martin's aunt, employs an unconventional method to test Martin's blood phobia, and Bert tries to evict a squatter from his trailer park.

In episode six, Martin and Louisa travel to London, where Martin is due to deliver the keynote address at the Imperial College Obstetrics Conference. Things don't go to plan, however, as Martin accidentally gets headbutted while saving a choking woman, and his ex-girlfriend Sophie causes a scene at the cocktail reception.
